FATCA Terms and Conditions:

1. I/We certify that the information provided on this form is accurate and complete to the best of my/our knowledge, including the taxpayer identification number of the applicant.

2. I/We authorize “RA SASIKUMAR PEYYALA: INH000019327” to collect, store, communicate, and process any changes in the FATCA status reported by an applicant to the relevant authority as required by law.

3. I/We will inform and update “RA SASIKUMAR PEYYALA: INH000019327” of any changes in FATCA status within 30 days of such changes.
